Pharmacodynamics and pharmacokinetics

Nicerium tablets have a neuropharmacological effect: they improve the level of glucose in the human brain, the synthesis of proteins and nuclein. acids, affect various neurotransmitters .

The active substance, nicergoline, undergoes rapid and extensive metabolism after oral administration. The resulting metabolites exhibit activity in the central nervous system at several levels. Long-term use increases the activity of choline acetyltransferase and acetylcholinesterase and the density of muscarinic receptors .

In addition, the activity of translocation protein kinase isoforms , resulting in a decrease in the production of pathological forms of β-amyloid . Nicerium protects nerve cells from death due to oxidative stress or apoptosis, activates detoxification enzymes, which guarantees an antioxidant effect . At the molecular level, there is an attenuation age-dependent nitric oxide synthetase messenger RNA improves cognitive function in general.

Pharmacokinetic properties and indicators

Nicergoline undergoes rapid and almost complete absorption when taken orally and has an absolute bioavailability of approximately 5% due to high hepatic clearance and first-pass metabolism. Subsequently, rapid and extensive distribution in tissues occurs and V distribution is more than 105 liters. Extensive protein binding in plasma occurs with greater affinity for α-acid glycoprotein compared to serum albumin . The drug is excreted in the urine mainly by excretion, with feces - only 10–20%. Therefore, patients with kidney problems are prescribed smaller doses of nicergoline .

Pharmacokinetics are linear if the dose taken ranges from 30–60 mg of nicergoline .

Instructions for use and dosage

Sugar-coated tablets, Nicerium:

The drug is intended for oral use. It is recommended to swallow film-coated tablets whole, without chewing or crushing, with a small amount of liquid. The drug should be taken before meals. The daily dose of the drug is prescribed in one dose or divided into several doses throughout the day. The duration of the course of treatment and the dose of the drug are determined by the attending physician individually for each patient.

Adults are usually prescribed the drug in an initial daily dose of 20-30 mg (2-3 tablets of Nicerium 10). If the therapeutic effect is not sufficiently pronounced, the dose is increased to 6 tablets of the drug Nicerium 10.

The duration of treatment with the drug is usually at least 6 months. The maximum therapeutic effect of the drug develops 3 months after starting the drug. Patients with impaired renal function require dose adjustment of the drug.

Nicerium 30 Uno capsules:

The drug is intended for oral use. It is recommended to swallow the capsules whole, without chewing or crushing, with a small amount of liquid. The drug should be taken before meals. The daily dose of the drug is prescribed in one dose or divided into several doses throughout the day. The duration of the course of treatment and the dose of the drug are determined by the attending physician individually for each patient.

Adults are usually prescribed the drug in an initial daily dose of 30 mg (1 capsule of Nicerium 30 Uno). If the therapeutic effect is not sufficiently pronounced, the dose is increased to 2 capsules of the drug per day. The duration of treatment with the drug is usually at least 6 months. The maximum therapeutic effect of the drug develops 3 months after starting the drug. Patients with impaired renal function require dose adjustment of the drug.

Indications for use

  • Acute and chronic disorders in cerebrovascular metabolism that developed as a result of thrombosis , atherosclerosis , embolism or transient cerebral circulatory disorders.
  • Acute and chronic metabolic disorders of peripheral vessels, including both organic and functional disorders of the arteries of the extremities, in Raynaud's disease and other syndromes associated with circulatory disorders.
  • Arterial hypertension (as an additional treatment).

Nicerium Uno is used

In patients in the period after a stroke , with vascular dementia ( multi-infarction ), degenerative conditions associated with dementia (for example, Alzheimer's type or disease ).

Indications for use of the drug Nicerium

Acute and chronic cerebral circulatory failure (including cerebral atherosclerosis, transient cerebrovascular accidents, thrombosis and embolism of cerebral vessels); migraine; Raynaud's syndrome; AH (arterial hypertension) (as an adjuvant); as a supporting agent for elderly people with cerebroorganic psychosyndrome with the following main symptoms: decreased concentration, weakened memory, disorientation, sleep disturbances, Alzheimer's disease, Parkinson's disease, vascular and mixed dementia.

Contraindications

  • hypersensitivity to nicergoline , other components and ergot alkaloids ;
  • recent myocardial infarction
  • orthostatic hypotension;
  • severe bradycardia ;
  • acute bleeding;
  • increased risk of collapse ;
  • vascular atherosclerosis;
  • angina pectoris;
  • therapy with sympathomimetics or stimulants of α- or β-receptors .

Side effects

  • constipation, diarrhea, vomiting, nausea, increased acidity of gastric juice, abdominal pain;
  • arterial hypotension , angina attack , tachycardia , “cold” extremities;
  • dizziness, fainting, headache, drowsiness, insomnia, confusion;
  • manifestations of an allergic reaction: skin itching, rashes, angioedema ;
  • ejaculation disorders in men;
  • fever, hot flashes, increased sweating;
  • an increase in the concentration of uric acid in the bloodstream.

Nicerium, instructions for use (Method and dosage)

The tablets should be swallowed whole, washed down with a certain amount of water (for example, 1 glass) before meals.

The recommended daily dose is 15–30 mg, divided into 3 times, the interval should be maintained the same. The duration of treatment is determined individually based on the specific clinical picture.

Daily dose of Nicerium Uno: 1 capsule 1-2 times a day, maximum - up to 60 mg.

The therapeutic effect increases gradually, because long-term therapy is carried out; the attending physician evaluates the patient’s condition every 6 months and, on the basis of this, decides on the advisability of further taking the drug.

For vascular disorders of the inner or ear of the eye

The recommended daily dose should be in the range of 15–30 mg. The drug is taken in the morning before breakfast, swallowed whole and washed down with liquid.

Use of the drug Nicerium

Capsules and tablets should be swallowed whole with plenty of liquid (such as water) before meals. The usual daily dose is 20–30 mg of nicergoline, which can be taken as a single dose or divided into several single daily doses. If the desired therapeutic effect is not achieved with this dose, the daily dose can be increased to 60 mg of nicergoline. After a noticeable improvement in the patient's condition, the dose of the drug may be reduced. In patients with impaired renal function (serum creatinine level above 200 mg/ml), the dose should be reduced. Since the therapeutic effect increases gradually, it is recommended to take the drug for a long time (at least 3 months), but at least every 6 months the need for further treatment with the drug should be assessed.

Overdose

Manifests itself in the form of a significant decrease , ataxia , shortness of breath and tremor .

Method of treatment

  • gastric lavage;
  • Activated carbon;
  • maintaining vital functions;
  • cardiovascular system monitoring;
  • appropriate symptomatic treatment: to reduce blood pressure , indirect or direct α-sympathomimetics , for vasospasm - vasodilators ( calcium antagonists , Theophylline , Papaverine ).

Interaction

  • In combination with antihypertensive and cholinomimetic drugs, potentiation of their effects is possible.
  • With drugs metabolized by the cytochrome P4502D6 , with Quinidine sulfate , most antipsychotics , Clozapine , Risperidone , Haloperidol , Thioridazine prescription and treatment are carried out with caution.
  • When used in parallel with acetylsalicylic acid, bleeding time may lengthen.
  • With anticoagulants or antiplatelet agents, regular monitoring of blood coagulation parameters is necessary.

Interaction with other drugs

The drug should be used with caution together with:

  • antihypertensive drugs (Nitserium 30 UNO® may enhance the effects of these drugs);
  • drugs that are metabolized by the cytochrome P450 2D6 system, since it is impossible to exclude interaction with these drugs (such as quinidine, most antipsychotics, including clozapine, risperidone, haloperidol, thioridazine);
  • acetylsalicylic acid (bleeding time may lengthen);
  • drugs that affect the metabolism of uric acid (metabolism and excretion of uric acid may change).

Nicerium UNO 30 cannot be used simultaneously with drugs that excite the central nervous system, α- and β-adrenergic agonists. Since nicergoline increases the effect of anticoagulants and antiplatelet agents, when they are used simultaneously with nicergoline, it is necessary to monitor blood coagulation parameters.

The drug may enhance the effects of cholinomimetic drugs. Nicergoline may potentiate the effects of beta-blockers on the heart.

special instructions

Nicerium tablets use the dye E110 - sunset yellow , which can cause allergic reactions and bronchial asthma in people with particular sensitivity to this compound; most often, allergies occur in people with hypersensitivity to acetylsalicylic acid .

Due to the constituent component of the drug - sucrose , it cannot be prescribed to individuals with rare hereditary forms of galactose intolerance , lactase deficiency , sucrase-isomaltase , glucose-galactose malabsorption .
